LT Home Healthcare provides Primary Home Care (PHC) services – a Medicaid entitlement program designed for Texas residents who need assistance with activities of daily living. As it is a “Medicaid entitlement program”, it means that all eligible clients will surely receive services under the program. To be eligible, an individual must be 21 years old and above and requires assistance to perform at least one activity of daily living. Their functional limitation must be due to a medical condition and it must be documented by a qualified medical professional.
Services include:
- Assistance with Activities of Daily Living (bathing, grooming, dressing, toileting, etc.)
- Assistance with the Instrumental Activities of Daily Living (housekeeping, meal preparation, etc.)
- Transfer and Positioning Assistance
- Transportation Assistance
- And more
Services under the Primary Home Care (PHC) program may be provided by the state or clients can choose their own care provider.
